Electronic Health Records - Physical Medicine and Rehabilitation

Mr. Thomas Johanson, 78 years old, Muscle Spasm — Chronic — this Electronic Health Record redesign turns a patient file into an interactive 3D diagnostic experience that makes clinical data genuinely readable.

UI/UX Design — Electronic Health Records

Physical Medicine & Rehabilitation · EHR System · Medical Dashboard

A visually ambitious Electronic Health Record interface design for a Physical Medicine and Rehabilitation department — combining patient profile data, 3D anatomical visualization, and multi-specialty navigation in a single, tablet-optimized dashboard.

The left panel presents the patient profile: Mr. Thomas Johanson, DOB March 24, 1944 (78 years), Male, Blood Type O (RHO positive). Below the patient card, four quick-access modules — Medical History Check-up, Vital Sign Check-up, Physical Check-up (active, teal), and Supporting Check-up — organize the clinical workflow into task-based navigation that maps how physical medicine practitioners actually move through a patient encounter.

The dominant right panel is the design's defining innovation: a fully rendered 3D muscular anatomy model displayed from the posterior view, with interactive diagnostic pins marking clinical findings on the body. A tooltip callout reads "Muscle Spasm — The forceful and involuntary contraction of a muscle — CHRONIC" with a "More details" CTA — transforming the anatomical model from a visual aid into a clickable diagnostic interface. The "Comprehensive health reports & analysis" header confirms this is a full clinical reporting system, not just a reference tool.

The bottom specialty carousel — Physical Medicine & Rehabilitation (active), Thoracic Cardiac and Vascular, Gastroenterology & Hepatology, Urology & Andrology — with attending physician avatars confirms a multi-disciplinary EHR platform serving coordinated care teams.

An EHR design that makes the body the interface — and puts clinical insight exactly where it belongs: on the patient.
